Here is a prime example of why leading second top (false card) is incorrect.

I do see what you stated in this hand -
YA, QS LEAD MADE NO SENSE? OH YA I WAS LOOKING FOR IT ON AN EARLIER TRICK! HOW BAD IS THAT?

That said, dealer makes a big mistake not leading trump to the 2nd trick, and still survives (Due TO THE QS LEAD)!

Played correctly, the dealer would have made his point if he leads trump to the 2nd trick! Or alternatively, he could have led the King of the same off suit as it is just like a trump lead.

Just an awful play, I sincerely hope that was a mis-click, there's no logic behind such a play.

Edit: Wait, is that a bot? Leading a losing suit in an A tripleton is definitely something a bot would do.

These were ranked games as we did not have 4 players to set an OE table. If it had been a bot, I would have shown the bots name.

Yes, it may have been a misclick. There is no way to know for sure, but leading to queen caused the partner to use a trump unnecessary.
